DOCUMENTS REQUIRED FOR EMPLOYMENT

Clinical applicants will have to provide the following documents:

• Resume
• NYS Nursing License
• Current NYS License Registration
• Diploma from Nursing School
• NPI # (National Provider Identifier)
• Malpractice Insurance
• Current CPR Certification Card
• Identification Documents for I-9
• Physical – signed and stamped by Medical Doctor (within the last 12 months)
• PPD – must indicate date implanted, date read and result (within the last 12 months or Chest X-Ray(CXR)
• Flu Vaccine – written documentation from Medical Doctor, stating reason, if any, for not taking flu vaccination
• Proof of Rubella Immunity (include lab report if possible)
• Proof of Rubeola Immunity (include lab report if possible)
