Our Mission

Family Pediatric Home Care believes in a family-centered approach to patient care. Our focus is in forming a partnership with each family to meet the critical care needs of the medically fragile patient and to strengthen the family dynamics. Our goal is to provide each patient with an exceptional nursing care plan to achieve the highest level of optimal wellness within the confines of his/her diagnosis. We have the capability to make each care plan flexible and personalized to maximize quality of care for the patient and the family.

Family Pediatric Home Care, a division of Tri-Borough Home Care, Ltd., is a trusted home care agency known for its highly specialized nursing care. Family Pediatric Home Care is one of two agencies approved and licensed by the New York State Department of Health to provide Home Care Services to critically ill children. Family Pediatric Home Care has over 15 years in the home care industry. We are accredited by Community Health Accreditation Program (CHAP) and pride ourselves in the delivery of exceptional nursing care to the critically ill in the comfort of their homes.

Family Pediatric Home Care provides home health care services 24 hours a day, 7 days a week. We bridge the gap between hospital and home by forming a partnership with doctors, nurses, discharge planners and case managers so we may provide a smooth transition for the patient and their family. We strive to minimize the anxiety of going home by maintaining continuity of care. Training and education continue in the home for the patient and family. Communication is maintained with the patient’s multi-disciplinary health team throughout any clinical changes.
